There are still a few forms that you may still need to print out. If you are doing ANY training with us, you will be asked to fill out a medical and you MAY need to see a doctor before you will be allowed to participate. Please fill out the medical ASAP so you are not disappointed on your dive day.
Please email digital copies of your medical to [email protected] as soon as you can and then bring the original with you for your class.
Referral Students
If you are a referral student please send copies of all your paperwork, logbook, or anything else your original instructor has given you to [email protected] at least 1 week in advance of your scheduled dives. This is very important so we can make sure everything is in order before you come to St. John.
